人工晶体有前房型、虹膜固定型、虹膜- 囊膜固定型及后房型。

最常用的是后房型人工晶体,其优点为:后房型人工晶体引起的像差小,为白内障术后提供了最好的屈光矫正;重量轻、固定好的人工晶体能位于正常晶状体的生理位置;远离角膜和避免与虹膜发生摩擦,减少了术后角膜和虹膜等并发症;对瞳孔的影响小,术后可自由扩瞳,利于观察眼底。在选择人工晶体前,应作眼球超声波检查及角膜屈光度的测量。超声波检查是人工晶体植入术前的常规,超声波除了了解眼球后段的病变,如玻璃体积血、混浊、视网膜脱离、眼内肿瘤、视盘异常和有隆起边缘的视网膜巨大裂孔等病变外,还可进行眼轴长度的测量。眼轴长度是指角膜顶点到黄斑的长度。用超声波测量眼轴的长度,其准确性可在0.1mm 内。眼轴的测量务求准确,因为眼轴误差1mm,人工晶体度数的误差将达2.5D。A 型超声波测量眼轴有两种方法:直接法(接触式或压平式)和间接法(浸渍法)。用直接法测量眼轴时应注意不要压陷角膜。眼轴测量后再进行角膜屈光度的测量,根据上述测量结果应用SRK Ⅱ公式计算出准确的人工晶体度数,使术后能够达到或接近术前所希望的屈光状态。当然术者应根据患者的年龄、职业需要、生活习惯、过去戴镜史及对侧眼的屈光状态决定植入人工晶体的度数。
